Plan at a Glance: Members (1) may apply for a benefit amount up to $1,000,000,in $25,000 increments. (2)Coverage can continue up to the age of 75. Your spouse/domestic partner may apply for up to $500,000, in $25,000 increments. (2) Your spouse/domestic partner’s coverage amount cannot exceed 50% of your coverage or $500,000. (2) Coverage reduces at age 65 to 50% of the face value amount (of the amount in-force at age 64) and at age 70 to 50% of the face value amount (of the amount in-force at age 69) before terminating at age 75. (3) Refers to your unmarried, dependent children through age 25. (4) Will Preparation Services and Estate Resolution Services are offered by Hyatt Legal Plans, Inc., a MetLife company, Cleveland, Ohio. In certain states, Will Preparation Services and Estate Resolution Services are provided through insurance coverage underwritten by Metropolitan Property and Casualty Insurance Company and its affiliates, Warwick, RI. These services are provided at no additional cost to those who purchase Supplemental Life Insurance only. (6) The Accelerated Benefits Option is subject to state regulation and is intended to qualify for favorable federal income tax treatment, in which case the benefits will not be subject to federal income taxation. In some cases a medical exam may be required (at no cost to you). When you apply, simply answer the health questions. Depending on the amount applied for, a paramedical exam and blood test may be required, which will be scheduled at your convenience. Even if you have a health condition, you still may qualify.
